IMPORTANT NOTICE FOR PARENTS/GUARDIANS OF STUDENTS IN GRADES 5  

New immunization rules for the State of New Hampshire require that your child have proof of Varicella (Chicken pox) PRIOR to entering 6^th grade.

 Proof is considered:

        1. A copy of your child’s immunization record from his/her health care provider showing the month, day and year of receiving the immunization.

                                                OR

        2. A medical history of having the disease as verified by your child’s health care    provider.

                                                OR      

3.       A statement from you indicating the date that your child had the disease.
