News from the Nurse Why my child is was not in school

Some information you should know
- Medication that is given by the School Nurses MUST have a signed Physicians Note and a Hold-Harmless form filled out by the parent.
- All school children will be screened for head lice two times a year and as needed. PLEASE let the nurses know if you are treating your child at home. All information is strictly confidential.
- Dr. Quimby administers eye screening tests to all second graders. Mrs. Richardson will be screening the fourth grade and all students that are referred by either teachers or parents.
- Scoliosis screening is done in fifth grade.
- It is very important to notify us if your student has any communicable disease. These include chicken pox, fifth's disease, lice, and strep throat to name a few.
- All fractures should be accompanied by a doctor's note.
